| The below forms are used by nationals to enusure compliance with our national policies, with the policies of our underwriters, and to assist in maintaining our brotherhood. Please ensure that they are fully completed and submitted on a timely basis. | ||||

| Record of Affiliation | This form must be filled out completely and legibly by each pledge, signed and returned to the National Headquarters. | Immediately upon pledging. | ||
| Pledge Drop | Use this online form to notify the National Headquarters upon a pledge quitting or being blackballed from the chapter. | Immediately on change of status. | ||
| Initiation Notice | Use this form to notify us that you will be initiating pledges. | One week before scheduled initiation. | ||
| Hazing Acknowledgement | The President and Pledgemaster should sign and complete this form and return it at the start of each pledge period. | At the start of each pledge program. | ||
| Graduation Report | Should be completed with the names and current address of any brother graduating from the chapter. | At the end of the term. | ||
| Expulsion Notice | Please complete a copy of this form for any member de-brothered. | Immediately | ||
| Election of Officers | Please complete and return to the National Headquarters whenever the chapter changes officers. | Within one week of elections. | ||
